Case # 9 Schizophrenia: Paranoid Type Twenty-five year old Bill McClary had been living with his sister Colleen’s family for eighteen months before seeking therapy. Colleen and her family had been concerned with Bill’s peculiar behavior, and social isolation. The behaviors that were concerning them were Bill’s constant daydreaming, talking to himself, and saying things that made little sense. During the first interview session, Bill acted shy and his therapist noted that Bill occasionally blinked and shook his head as though he was trying to clear his thoughts. When the therapist asked Bill about that behavior, Bill denied any significance. Bill admitted to several momentary and detached bi-sexual relationships, however he was concerned about the consequences of his homosexual encounters. Changing the subject, the therapist asked Bill about his daydreaming, which he explained were frequent intrusive thoughts disturbing his belief system and he felt compelled to repeat a sequence to correct them. Bill referred to this behavior as “scruples.” After receiving a call from Bill’s concerned brother-in-law, the therapist began to question Bill about his extensive delusional belief system. Bill’s belief was that family and friends had recorded a high-grossing movie about his homosexual experiences, giving its profits to the Irish Republican Army to buy weapons, reinforcing his feelings of guilt.
