My husband, John, recently died. I am the beneficiary of the above-referenced account. The purpose of this letter is to roll over my husband=s IRA into my Fidelity IRA Account 532 as follows: I have enclosed the signed Fidelity IRA and Inherited IRA Application, a certified death certificate for my husband and a Fidelity Address Change form. My son, John, is an attorney. Please discuss my account with him or with Cindy Koepf of his office if they call you. You may call either of them at if you like. You may also discuss my account with my son, Kevin O=Grady. Thank you in advance for your prompt attention to this matter. At the hearing on the petition to administer the above referenced estate on August 8, 2002, you continued the matter until September 12, 2002 to give the parties time to attempt to reach a settlement.
